Jean Polser leads Neighborhood Family Practice, a Federally Qualified Health Center [FQHC] with 3 locations on the near west side of Cleveland. It strives to provide a high quality and personal approach to family medical care. All patients are welcome – regardless of ability to pay. Encore Fellow Elisa Ross, M.D. used data to improve patient care outcomes, increasing cancer screening rates by 50%.
